High Country Health Care, PC (HCHC) is a multi-site, multi-specialty, physician-owned medical group located in Summit and Park Counties in Colorado’s ski country. Formed in March of 1996 as the result of the formal merger of five small practices, the group includes 10 family practitioners, 4 Ob/Gyn’s, 2 pediatricians, a primary care physician assistant, a certified nurse midwife and 2 family nurse practitioners.
